WEST WALES CARE PARTNERSHIP
Your opportunity to help transform health, care and support in West Wales.
Welsh Government’s national plan for health and social care, A Healthier Wales, was launched in June 2018 and sets out its blueprint for the transformation and integration of  health, care and support throughout the country.
The West Wales Care Partnership (WWCP) brings together agencies across health, local government, the third and independent sectors and has responsibility for delivering this transformation within the region, under the direction of the Regional Partnership Board.

Our Healthier West Wales programme sets out an ambitious plan for whole system change and has drawn in £12m of new transformation funding to the region to support that change. This complements the health and care strategy recently adopted by Hywel Dda University Health Board A Healthier Mid and West Wales: Our Future Generations Living Well and a comprehensive programme funded through Welsh Government’s Intermediate Care Fund. Together these will deliver radical and lasting change improving the health and wellbeing of people across our communities.

The Regional Partnership Unit sits within Carmarthenshire County Council and provides strategic and programme support to the WWCP, facilitating cross-agency working to implement its priorities.

This exciting new position will provide robust financial accountability both to regional partners and Welsh Government. Leading on financial planning, profiling and reporting across the Transformation and Integrated Care Funds, you will provide high quality support to senior leaders and play a key role in ensuring the overall success of transformation activity in West Wales.
