This is an exciting opportunity to join our care team in a critical leadership role as our Service Coordinator assisting the Care and Support Team Manager to provide line management to the care team. The service provides Care and Support within an independent living scheme for tenants who are aged 55+. The Service Coordinator will take a leading role in the day to day running of the team.

This position is ever evolving and requires an individual who is willing to work under pressure and think on their feet. As part of the role you will be expected to be part of the company’s on-call scheme. The post will involve at least one care shift each week and may involve some weekend working and sleep-ins.

The Service

You will be employed by Family Housing which is an ambitious and well-established independent housing association.  We have strong local roots, a reputation for providing quality homes, and a track record for making a positive difference in our local communities.

Your work base will be Maes Mwldan, Cardigan, which is an extra care scheme.  We provide Care and Support Services to the tenants who live within the scheme.  Care services are provided from 07:00 until 23:00 each day.  There is a sleeping staff member through the night.

We aim to provide a high quality service that encourages tenants to have independence and remain living in their homes for as long as possible.

About You

We are looking for an enthusiastic individual, with extensive knowledge of care.  A basic understanding of the Supporting People Framework would be desirable.

You will be a good team player who can help the team maintain a positive attitude in order to provide a high standard of care.

You must have excellent communication skills and an ability to remain calm under pressure.

It is essential that you have good IT skills particularly, Word, Excel and Sharepoint.

You will be expected to ensure that the service always has  safe staffing levels and complies with in house policies and CIW regulations. You will have experience of care planning and risk assessment writing. We are looking for an individual who is flexible time wise as well as flexible in the way that they work. You will be positive, proactive and motivated with the ability to motivate others.

The ability to speak Welsh is desirable as many of our tenants speak Welsh as their first language.

You will hold a qualification in Health and Social Care Level 3 or above.

An enhanced Disclosure & Barring Service check is a requirement
